I'm surprised that the World's largest gold and copper mine with world's largest gold reserve (3 billion tonnes at 0.1%gold), and world's highest highest open pit mine did not make it to your list: Grasberg mine in Papua, Indonesia. It is located in the highest mountain range in Indonesia: Puncak Jaya is 4,800 m and has glacier that is now retreating. The environmental damage of the mine is unbelievable, not only in the mountain, but mostly because of the tailing water it dumps into the sea 80 km south.

I've been there and everything there were huge! they operate the largest trucks in the world with tires taller than a house. The open pit was orginally a 4,100 m mountain which has been mined down to less than 3,000 m and now with extensive underground mine. It is 5 km wide.
