My questions are about prostrating forehead should it touch the ground? A prayer mat on the ground I think. What if the prayer mat is not on the ground but on elevated flooring, such as in the house or masjid ( also the womans section are usually upper platform) and will the salat be acceptable?

All of the following should be touching the ground:
-Forehead
-Nose
-both hands
-both knees
-both feet
It shouldn't matter where the ground is, even if it is on a different floor like at the masjid you mentioned. The prayer will be accepted in this way inshallah.
