    Diabetes Mellitus, Insipidus or SIADH?
    You want an interesting bed time story? maybe not, it so happens you are reading this now because you have nothing better to do on a Thursday?

    whatever the case.. have you every wondered if that frequent thirst and urination has positively nothing to do with your typical diabetes? there is nothing typical about diabetes in its two forms.. but there are other diseases that can cause similar symptoms ..

    so you go to your dear ole doc and he runs a test and it turns out, it ain't diabetes mellitus.. don't take a sigh of relief yet.. he or she =) needs to rule out other causes..eh they call it diabetes insipidus because your urine is insipid not as in lacking taste, flavor or tang, doctors don't taste urine... at least not this last century anyhow, I won't digress to to irregular practices of the past of those dedicated stoics ...
    but indeed the urine is missing something essential, its 'osmolality' ... So you are putting out large amounts of free urine.. what does it all mean?

    well it could mean several things and it is really sort of like playing detective... maybe you are missing your ADH (anti-diuretic hormone) maybe you have a space occupying lesion .. yes that is bad.. invaders go every where including un chartered territory, maybe just a minor head trauma and that is certainly something to think about next time you head for your bike with out your helmet? Maybe even your hypothalamus is a little upset with your pituitary? indeed the two can be upset with one another and stop communicating.. eh maybe the problem is a little further down south and your dear ole doc takes a nice five minute break while he screams in the halls.. this guy is confusing me ..pls somebody call a doctor.. and the nurse comes in and slaps your doc back to his/her senses.. stating you are the only one on call this evening get a hold of yourself ...
    eh so s/he gets to thinking perhaps your psychiatrist just put you on some meds.. yes that is it .. it is always the other guys fault, when all else fails, someone else did you in (remember that) ..no matter it is time to decide what is troubling ya...

    so you think your doc is now being all hospitable when he invites you in for an over night stay in the three star hospital, but he has no good manners.. yes, you are in desperate need of a drink but he admits you and asks all the house staff not to offer you any water.. no he isn't being ungracious he wants to see how your body will react to it...now if you were just faking it .. yes there is such a thing as psychogenic diabetes insipidus perhaps I shouldn't let you in on the secret just in case you were suffering Munchausen or some other fictitious disorder wouldn't take advantage of this? I digress. .. got to keep alert it is almost isha time..but your body is smart, under lock and key, thus after a short while on no water, it can response in one of many ways...one, your ADH will shoot up and conserve water... your body will still keep on producing dilute urine and you'll really be desperate for some water.. or it will response appropriately but be slightly skewed to the right (on the graph that is) -- I won't tell you which is which, but those erudite in the midst, must have definitely figured it out?

    so what next? your doc gives you, your missing hormone or measures the level of the hormone.. and now you are living in harmony .. but what if your body doesn't respond to that either? well he or she will take a sigh of relief ... thank you God.. the answer is somewhere down south.. your body is communicating so well up above but your kidneys aren't responding to the message sometimes organs are upset with one another...now that is a topic for another day and another thread but at least now you are on the path of recovery...

    wait wait.. didn't we say we'd discuss SIADH? well there is no way I am going into any more details.. but let's just say instead of all that free water out, all the free water is retained and you produced very hypertonic urine anything but flavorless ... causes are similar you know Mr hypothalamus and Mrs. pituitary have had an impasse, meds from your dear ole doc don't blame the psychiatrist this time, but the oncologist.. some lesion, some trauma some tumor etc .. but it could also be due to lung disease COPD, granuloma, abscess joe mama ....all that water retained dilutes the balance of electrolytes in the kidneys and of course needs to be attended to and corrected at once...

    that is your body at a glance.. don't get paranoid now on your way to the bathroom?

    You never know when your body is gonna betray you ey?.. honestly be good to it and it will be good to you.. like a well oiled machine.. the rest is all in the hands of Allah!

    if I may, make an assimilation... contrasting between religious duties and duties in your every day life... you ( I don't mean anyone here just a general you) don't want to get up and pray or fast in Ramadan, dressing conservatively is 'ugly(fying)' look at all those natural endowments you'd rather share with the ladies, you'd rather watch TV, go dancing, hang with your poker buddies and eat lots of pork rinds, fried chicken and wake up late...

    Well one day you wake up in need of a quintuple bypass , you get fired from the job because you were slacking off getting high strolling in late, you can't pay mortgage 'cause you gambled it all away trying to double your winnings, and you lost your wife because you were ogling every chick down on 42nd street..

    You'd think well being square is no fun.. I'd rather disco than wake up at 5 to jog, I'd rather listen to 'it is hard out here for a pimp' than recite morning prayer.. I'd rather eat fried chicken to oat meal.. I'd rather stroll in to work at 11 'cause I was up all lost night, I'd rather drink my way to wernicke encephalopathy and boerhaave's syndrome than take refuge reciting the book of Allah.. one life style will catch up with you here, and the other will catch up with you in the here after..

    the moral(s) of the story if ever there were any...
    1- there is so much going on right in your bodies that you don't think about or give thanks for.. and it is tireless and round the clock.. you take rest at the end of the day but it keeps working for you on its own volition.. imagine your heart saying.. I don't want to beat today, too tired, been doing it since 21 days in utero.. I think I'll nap now a while?
    2-There is potential to abuse a good thing, and it will ruin you sooner or later...
    3-Having what others deem as a square life style, might actually lead you down the path of victory immediately and in the life to come..

    of course you were always free to draw your own conclusion...

    Just a gentle reminder. Most of you are fairly young, it is difficult to even imagine that what you do to your body today, will still be having an effect until the day you die.

    Sadly, as time goes on, each incident of maltreatment, abuse, indulgence is going to become compounded. What makes you feel good today, can cause extensive pain at the time of your life you should be able to relax and enjoy your life.

    Most diseases and conditions can be treated or prevented. Begin doing so today, early detection, proper care and treatment as needed can go a long way to assure you a pleasant life when you get older.


    You only get one body in this life and it has to last you a lifetime.

    Subha'nallah, there is so much work been done in the body we are completely oblivious to. If we did create the actions we would have full knowledge and power of doing that action; however if you ask anyone that walks from A to B, the walk it self compromises a series of interspersed stops along with movements that oscillate between faster ones and slower ones, Meanwhile the one walking is totally oblivios to such subtleties. The is basic level, it gets even more complicated and confusing if one where asked about the internal movements of limbs and organs while walking, talking, striking etc and what he needed interms of micromovements of his musceles and elasticity of his cartillage and nerve impulses; these questions make it even more apparent that one knows nothing regarding their particular actions...

    Prophet said,

    True that oh: Dear friend of mine was saying how his uncle damaged his leg and how it got worse as time went on. Now he is stressed about how he got that swell on his leg since the last time he played football and started limping May Allah(SWT) give him a speedy recovery.

    Today I was going to start another topic about the thyroid gland... its horomones, T3, T4, TRH, TSH..diffrentiating between hyperthyroidism, hypothyroidism, surriptitious use of thyroxine... to all the different forms of disease that can occur from Hashimoto's Thyroiditis, to riedel's thyroiditis to graves disease, sick thyroid symptom, to Euthyroid and how that all differs even in the pregnant state.. the different types of cancers, including papillary, medullary, follicular and anaplastic and we are just talking about one little organ at this tiny organ at the base of the neck
