Here are some steps of the prayer that could get you started on putting the words to the actions...These are only the fardh prayers, so you'd still have to incorporate the sunnah prayers, which are generally prayed in the same manner.
Of course there are many more, but he was one of my first favorites.
If you take some time learning the steps of salaah and incorporating the quran with it, it will make your prayers easier and easier and you'll start doing everything by heart inshallah. If you don't know how to do something, feel free to ask!
I believe what you are looking for is a visual flash/video presentation showing how to do the ablution/wudu and how to pray properly as well as what to say. Here is a good link for that:

As far as I know there is no such program out there. If you want to learn salaah then do it this way.
Watch the flash presentation from the link i gave you to see how a 2 rakah, 3 rakah and 4 rakah is done. Learn what is said at each position and what are the movements of each set of rakahs.
Start memorizing and learning your surahs as well as what is said at each position the movements. Memorizing and knowing them will make it easier for you not to think what to say. so salah is 2 part, learn the movement and learn the stuff to say. You can learn what to say from the presentation or the online book listed on that site and learn surah fatiah and other small surahs here: http://mounthira.com/
I'll tell you what you need to know for prayers, because essentially there are some mandatory acts, and some optional acts (such as certain du'as)
Some mandatory memorizations are:
1. Al-fatiha - You HAVE to recite this in every rakaah, you can find this very easily in any of the links above.
2. A short surah - This can be any part of the quran, some people like to recite a short surah (most are found at the end of the Quran) or you may recite a few verses of a longer surah, but it's all your choice in the end.
3. Subhana rabiyal 'adheem - you say this 3 or more times in rukuu'
4. Sami Allahu liman hamida (this is said when you're standing after ruku')
5. Rabanna lakal hamd (said before sujuud)
6. Subhana rabiyal a'la (said 3x or more while in sujuud)
7. The tashahud (said in the 2nd and final sujuud, you can also find this in the above praying videos..it starts out like "at-tahiyatu lillah wasalawat watayibat..")
8. Salat 'alan Nabi - this is said in the final sujuud, found in the above links as well, starts out like "allahuma salee 'ala muhammad wa 'ala alee muhammed..."
this is what is required for you to have memorized..i didn't include the littler things such as takbir (allahu akbar) and ta'awuz (a'udhu billah mina shaytan arajeem) and tasmiah (bismillahi rahman araheem) which are very very important as well, cuz I assumed you might already know those?
