This isn't 'how to pray', just a meaning in English of what we say in prayer, and it is surely not limited to just this. Wa'salamu alaykum wa Rahmat Allahi wa Baraktuhu

I have a booklet on how to pray and it includes a part after "Innaka hamee-dun Maajeed":
Rabbij-Alni muqimas-salate wa min zurriyati - Our Lord! Make me and my children steadfast in prayer;
Rabbana wa taqabbal dua rabbigh firli - Our Lord! Accept the prayer! Our Lord! Forgive me.
Wa liwalidayya wa lil mu'mineena - And my parents and believers
yauma yaqoomul hisab - on the day of judgement.
do you have any idea where this may be from? Is it perhaps Shi'ia?

This is rabbi jalni dua, it's just one of any duas you can recite towards the end. It's not shia.

This is a Du'a from the Qur'an. Surah Ibrahim (14), Ayat 40 and 41.
