Interesting survey. Only suggestion is you should have had something about religious affiliation. We have members here from many different faiths and even Atheists and Agnostics.
It is highly doubtful your survey will reflect the general views of Muslims. You also can not go by name as we have many Muslims without Arabic names and non-Muslims with Arabic names.
Might have been good to mix the questions a little.
