Diabetes Mellitus, Insipidus or SIADH?
You want an interesting bed time story? maybe not, it so happens you are reading this now because you have nothing better to do on a Thursday?
whatever the case.. have you every wondered if that frequent thirst and urination has positively nothing to do with your typical diabetes? there is nothing typical about diabetes in its two forms.. but there are other diseases that can cause similar symptoms ..
so you go to your dear ole doc and he runs a test and it turns out, it ain't diabetes mellitus.. don't take a sigh of relief yet.. he or she =) needs to rule out other causes..eh they call it diabetes insipidus because your urine is insipid not as in lacking taste, flavor or tang, doctors don't taste urine... at least not this last century anyhow, I won't digress to to irregular practices of the past of those dedicated stoics ...
but indeed the urine is missing something essential, its 'osmolality' ... So you are putting out large amounts of free urine.. what does it all mean?
well it could mean several things and it is really sort of like playing detective... maybe you are missing your ADH (anti-diuretic hormone) maybe you have a space occupying lesion .. yes that is bad.. invaders go every where including un chartered territory, maybe just a minor head trauma and that is certainly something to think about next time you head for your bike with out your helmet? Maybe even your hypothalamus is a little upset with your pituitary? indeed the two can be upset with one another and stop communicating.. eh maybe the problem is a little further down south and your dear ole doc takes a nice five minute break while he screams in the halls.. this guy is confusing me ..pls somebody call a doctor.. and the nurse comes in and slaps your doc back to his/her senses.. stating you are the only one on call this evening get a hold of yourself ...
eh so s/he gets to thinking perhaps your psychiatrist just put you on some meds.. yes that is it .. it is always the other guys fault, when all else fails, someone else did you in (remember that) ..no matter it is time to decide what is troubling ya...
so you think your doc is now being all hospitable when he invites you in for an over night stay in the three star hospital, but he has no good manners.. yes, you are in desperate need of a drink but he admits you and asks all the house staff not to offer you any water.. no he isn't being ungracious he wants to see how your body will react to it...now if you were just faking it .. yes there is such a thing as psychogenic diabetes insipidus perhaps I shouldn't let you in on the secret just in case you were suffering Munchausen or some other fictitious disorder wouldn't take advantage of this? I digress. .. got to keep alert it is almost isha time..but your body is smart, under lock and key, thus after a short while on no water, it can response in one of many ways...one, your ADH will shoot up and conserve water... your body will still keep on producing dilute urine and you'll really be desperate for some water.. or it will response appropriately but be slightly skewed to the right (on the graph that is) -- I won't tell you which is which, but those erudite in the midst, must have definitely figured it out?
so what next? your doc gives you, your missing hormone or measures the level of the hormone.. and now you are living in harmony .. but what if your body doesn't respond to that either? well he or she will take a sigh of relief ... thank you God.. the answer is somewhere down south.. your body is communicating so well up above but your kidneys aren't responding to the message sometimes organs are upset with one another...now that is a topic for another day and another thread but at least now you are on the path of recovery...
wait wait.. didn't we say we'd discuss SIADH? well there is no way I am going into any more details.. but let's just say instead of all that free water out, all the free water is retained and you produced very hypertonic urine anything but flavorless ... causes are similar you know Mr hypothalamus and Mrs. pituitary have had an impasse, meds from your dear ole doc don't blame the psychiatrist this time, but the oncologist.. some lesion, some trauma some tumor etc .. but it could also be due to lung disease COPD, granuloma, abscess joe mama ....all that water retained dilutes the balance of electrolytes in the kidneys and of course needs to be attended to and corrected at once...
that is your body at a glance.. don't get paranoid now on your way to the bathroom?
