Insha'Allah, if I'm correct it's from Mishkat al Masabih, narrated by Imam al Tabrizi.

I don't know which number it is though.

اللهُمَّ فأَيُّمَا مُؤْمِنٍ سَبَبْتُهُ فَاجْعَلْ ذَلِكَ لهُ قُرْبةً إليكَ يَوْمَ القِيَامةِ.

Shouldnt there be a Jar Majroor there "Alaa" between the ilayka and yawm ul qiyamaah?

You mean Harf Jarr right?

No, i don't think it's necesessary, however if I were to put a Harf Jarr there I'd put "fee" there.
