Alhamduilliah. Islam is easy to enter. Simply just say I believe that there is no god but Allah and that Muhammad is Messenger of Allah
In arabic : La ilaaha illal laah Muhammad Rasuul Allah
Islam is based on five pillars there is none worthy of worship except Allah, to pray salat, to pay zakaat, to fast in month of Ramadan and to perform Hajj (pilgrimate to Mecca), and then 6 basic belief, to believe in One and Only Allah, to believe in His angels, His holy books (Quran, Torah and Injil (Gospel), Psalam), His Prophets (Adam to Muhammad peace be upon them), Resurrection after death and then Divine Decree, which good and bad things are from Allah.
Allah knows best. Please say shahadah. Time has no excuse.

All what you can do is to recite the shahada: there is no god but Allah/ One God and Muhammad is His Prophet in Arabic and then you have automatically reverted to Islam.
You don't convert to Islam, you are only reverting to Islam. Because, you were born Muslim but your parents corrupted you with false religion and now you are reverting back to your true religion which is Islam.
